The two main paths
Medicare Supplement vs. Medicare Advantage
Most people choose between these two routes. Here's a quick comparison.
Medicare Supplement
Pairs with Original Medicare (Parts A & B) to cover the costs Medicare doesn't cover (deductibles, coinsurance, and copays). Add a Part D drug plan.
- ✓See any provider nationwide that accepts Medicare
- ✓Very predictable monthly costs
- ✓No referrals needed
- •Higher monthly premium than MA
- •Stand-alone Part D plan needed for Rx
- !Guaranteed-issue only during your 6-month Medigap Open Enrollment Period at 65. After that, insurers in most states can use medical underwriting and may decline or charge more.
Medicare Advantage
An all-in-one plan from a private insurance company. Bundles Parts A, B, and usually D, plus extras like dental, vision, and hearing.
- ✓Often $0 monthly plan premium
- ✓Extra benefits (dental, vision, hearing, fitness)
- ✓Annual out-of-pocket maximum
- •Provider networks (HMO / PPO)
- •Plan benefits and networks can change each year
- ✓No medical underwriting, ever. Switch Medicare Advantage plans each year during AEP regardless of your health.
Why your choice at 65 matters more than people realize
When you turn 65, you have a one-time 6-month Medigap Open Enrollment Period where insurers must sell you any Medigap plan at the standard rate, regardless of your health. After that window, in most states, Medigap insurers can use medical underwriting and may decline you or charge more for pre-existing conditions. Medicare Advantage, by contrast, has no medical questions. You can move into Advantage from Original Medicare anytime during the Annual Enrollment Period (Oct 15 to Dec 7).
The catch: if you pick Medicare Advantage at 65 and later decide you want Medigap, you may not qualify for it. This is why a licensed agent should walk through both routes with you before you decide.
